“Partial extraction therapy – is there a tipping point?”
 

Implant dentistry has evolved from the Branemark era where implants were placed in a two stage approach with a period of undisturbed submerged healing. There has been a paradigm shift from these historical protocols developing into immediate placement along with more emphasis on the aesthetic criteria around dental implants.
 
When a tooth is extracted, the healing of the socket inevitably involves reshaping of the alveolar process with horizontal and vertical bone loss. Post-extraction immediate implant placement has been proven to be a reliable and safe process, with an osseointegration success rate similar to that of conventional implants, but it does not prevent bone remodeling after extraction. The socket shield technique was published for the first time in 2010 as a feasible technique to avoid buccal bone reshaping after tooth extraction, in combination with immediate implant placement. A thin buccal and interproximal layer or fragment of the tooth needs to be maintained in order to avoid buccal bone resorption. This may allow maintenance of the bundle bone vasculature allowing preservation of the labial plate and ridge dimensions.
 
This presentation will highlight the fundamental principles of this technique and share guidelines when this procedure might be indicated along with the associated pitfalls that may be encountered, providing insight into the protocols that some may consider controversial.
